Abstract
1/10: A case of disulfiram induced delirium Dr Parinita Dutta Senior resident, Department of Psychiatry, Gauhati Medical College and Hospital, Guwahati, Assam. BACKGROUND: Disulfiram was the first drug approved by FDA for treatment of alcohol dependence way back in 1949, but its use in modern days became limited due to the frequent occurrence of serious adverse reaction particularly with covert use. AIM: To report a rare case of delirium developed in a patient of alcohol dependence syndrome currently abstinent on being treated with disulfiram. METHODS: Clinical interview. RESULTS: We describe a case of a 37-year-old male who was started on disulfiram after the initial detoxification phase. But despite being abstinent the patient suddenly started showing confused behaviour, irrelevant talk, fearfulness, disoriented and hallucinatory behaviour, decreased sleep, restlessness, etc. Disulfiram was withheld immediately and the patient was treated with injectable benzodiazepine in in-patient setting. After discontinuing disulfiram all his symptoms improved dramatically. CONCLUSION: Disulfiram-induced delirium though unusual could be disruptive and severe, warranting immediate intervention. Email: [email protected] BACKGROUND & AIMS: Neurological Soft Signs (NSS) are non-localizable neurological abnormalities. They are indicators of neurological impairment and have been found in up to 50% of bipolar affective disorder patients suggesting neuro-developmental basis of illness. The study was carried out to describe NSS in patients with bipolar affective disorder and compare them with soft signs found in their first-degree relatives (FDRs) and healthy controls and to study the association of illness related factors with NSS found in patient. METHODS: The study included 25 patients, 25 FDRs and 25 controls. NSS were assessed using previously validated Neurological Evaluation Scale (NES). RESULTS: Maximum patients showed marked impairment in motor sequencing tasks. Rhythm tapping test was significantly associated with longer total duration of illness. Subscales of NES showed positive correlation with total score. Scores of FDRs were better than patients but worse than that of healthy controls. CONCLUSION: NSS manifest early in life, even before the onset of full-blown bipolar disorder symptoms; and hence, studying these markers may help identify individuals at risk or in the early stages of the disorder. While on treatment, he developed emergent mania with resolution of symptoms with SDA. Discussion: Handful of case reports of mania in patients of PTSD with depression. Baseline PTSD or Bipolar disorder increased the risk of developing the other illness. Several possibilities including shared genetic or environmental risks may explain these findings. There is negative impact of BPAD and PTSD on symptom burden and overall Quality of Life. Conclusion: It is important to evaluate the patient of BPAD and PTSD for the other condition. It is also important to develop strategies for secondary prevention among patients who experience either condition alone. Key words: BPAD, PTSD 04/30: When Smell Becomes the Shadow: Insights into Olfactory Reference Disorder – A Case Report Dr. Ashish Ranjan Panda Junior Resident, Indlas Hospital, Vijayawada Email : [email protected] Background : Olfactory Reference Disorder (ORD) is characterized by persistent preoccupation with the belief that one is emitting a perceived foul or offensive body odour or breath that is either unnoticeable or only slightly noticeable to others. Individuals experience excessive self-consciousness about the perceived odour, often accompanied by ideas of reference. Clinical Case : A 26-year-old male presented with complaints of visiting multiple dentists and otolaryngologists due to a perceived bad smell from his mouth and nose. He reported brushing his teeth four to five times a day and cleaning his tongue repeatedly, feeling that people around him were annoyed and constantly judging him for the past seven years. On physical examination, the patient was thin and malnourished. Mental Status Examination (MSE) revealed a dull mood, death wishes, and preoccupation with the bad smell. MRI of the brain and EEG were normal. On the Yale-Brown Obsessive Compulsive Scale – Olfactory Reference Syndrome (YBOCS-ORS), the patient scored 36, on Hamilton Depression Rating Scale (HAM-D), he reported a score of 26. He was prescribed fluoxetine 40 mg along with a low dose of olanzapine. Cognitive Behavioural Therapy (CBT) was initiated to complement the remission process. Conclusion: ORD is often under-recognized in patients seeking medical attention. It is important for physicians to assess the patient for relevant symptoms, inform them about the potential diagnosis, and refer them to a mental health professional for further evaluation and treatment. Email: [email protected] INTRODUCTION: Charles Bonnet Syndrome (CBS) is characterized by visual hallucinations which are “released” in visual cortex, as brain compensates for reduced input due to significant visual impairment. Insight is retained but it’s very distressing to the patient and may lead to depressive disorder or frank psychosis. CASE DESCRIPTION: 22 year old female with no previous psychiatric history and recently diagnosed with retinitis pigmentosa. She presented with complaints of seeing terrifying images, low mood and insomnia. On physical examination she had reduced visual acuity, MSE revealed visual hallucinations, dysphoric mood, preoccupation regarding loss of vision, visual hallucination, ideas of worthlessness and helplessness with grade 4 insight. She was managed with antipsychotics, antidepressants, supportive psychotherapy and family psychoeducation with concurrent treatment from ophthalmology department.
