Current view on the treatment of moderate and severe psoriasis

Abstract

Psoriasis is a chronic immune-mediated inflammatory disease with a predominant affection of skin and musculoskeletal system, and currently is a contemporary issue not only in dermatological practice. Doctors of each specialty eventually see this pathology. At the present stage of medicine development, psoriasis is recognized as an incurable disease often leading to complete disability and dysmorphia despite a wide range of treatment methods and facilities. We give an overview of the basic anti-inflammatory therapy used in the Russian Federation, which includes inhibitor of folic acid — methotrexate, synthetical retinoids — acitretin and genetically engineered biological drugs: infliximab, guselkumab, ustekinumab, apremilast and etc. Special emphasis is made on the treatment of moderate and severe forms of psoriasis combined with comorbid pathology, including HIV infection. The main components of psoriasis pathogenesis are also summarized in order to complete understanding of anti-inflammatory activity of one or another medicine. But the main difficulty for the clinician is the problem of choosing a treatment method in a particular situation, as well as the availability of medicines (such as acitrein and genetically engineered biological drugs) for most population.
