SGLT2 Inhibitor–Related Polycythemia in a Patient With Chronic Heart Failure: A Potential Severe Adverse Event

Abstract
According to the current heart failure guidelines, SGLT2 (sodium glucose cotransporter 2) inhibitors are increasingly prescribed in patients with heart failure (HF).1 However, SGLT2-inhibitors are known to increase hemoglobin (Hb) and hematocrit (Ht) levels, but literature on SGLT2-induced erythrocytosis is limited. We provide a case of a patient with chronic HF with severe polycythemia associated with dapagliflozin usage, suggesting the potential importance of hemoglobin and hematocrit monitoring.
