Abstract
Dr. Kinosian is Treasurer for the American Academy of Home Care Medicine, and a participant in the Complete Care Academy Collaborative, a High-Need Accountable Care Organization Realizing Equity, Access, and Community Health (HN-ACO-REACH). Drs. Boling, and Kinosian are uncompensated board members, representing their practices, for the Complete Care Academy Collaborative. Dr. Leff serves as a clinical advisor to Medically Home, Dispatch Health, MedZed, Patina Health, and the Chartis Group. He serves as a volunteer member of the Humana Multidisciplinary Advisory Board. Drs. Deligiannidis, Boling, Kinosian, and Taler are salaried employees of their respective institutions, which currently (Deligiannidis) or previously (Boling, Kinosian, Taler) participated in IAH.
